(To be submitted to the Admissions Office after passing the Entrance Examination; Numbers 1 to 8 need two (2) photocopies each)
- Form 138 (for Freshmen) OR Transcript of Records (for Transferees)
- Honorable Dismissal (for Transferees)
- Certificate of Good Moral Character
- Entrance Examination results & accomplished Application Form
- Evaluation of Academic Credentials (for Transferees)
- Birth Certificate
- Two (2) 2X2 ID pictures
- Two (2) long brown envelopes & long plastic envelopes
- One (1) long folder
APPLICATION PROCEDURE
- Fill-up an Application Form
- Attend Initial Interview by the Admissions Committee
- Fill-up the PHCM Personal Data Sheet & attach one (1) 2X2
ID picture
- Take the PHCM Entrance Examination (bring a short folder & a pencil)
