Admission Procedure for Freshmen

  1. Admission Center
    • Secure list of requirements and locator number.
  2. Cashier
    • Pay the locator number and present the receipt to the admission center for the entrance examination.
    • Pay the enrollment kit.
  3. Admission Center
    • Submit validated requirement and fill out admission form for encoding of 201 file.
  4. Dean's Office
    • Submit the validated and signed Dean's Copy envelope. Request for validation and assessment of subjects to be enrolled.
  5. Cashier
    • Pay the assessed tuition fee and other fees.
  6. Registrar's Office
    • Present receipt of payment. Get official registration form.
  7. ID Center
    • Present receipt of payment for ID card. Get official ID.



Admission Procedure for Transferees

  1. Admission Center
    • Secure list of requirements and locator number.
  2. Dean's Office
    • Present copy of grades from previous school for evaluation.
  3. Cashier
    • Pay the locator number and present the receipt to the admission center for the entrance examination.
    • Pay the enrollment kit.
  4. Admission Center
    • Submit validated requirement and fill out admission forms.
  5. Security Office
    • Submit self for interview.
  6. Admission Center
    • Present clearance from Security Officer for encoding of 201 file.
  7. Cashier
    • Pay the assessed tuition fee and other fees.
  8. Registrar's Office
    • Present receipt of payment. Get official registration form.
  9. ID Center
    • Present receipt of payment for ID card. Get official ID.



Admission Procedure for Old Students

  1. Dean's Office
    • Secure pre-enrollment form.
  2. Assessment Office
    • Present pre-enrolment form to be signed by the assessment officer.
  3. Library
    • Present pre-enrolment form to be signed by the librarian.
  4. SPS
  5. Present pre-enrolment form to be signed by the guidance counselor.
  6. Dean's Office
    • Present signed pre-enrolment form. Request for validation and assessment of subjects to be enrolled.
  7. Cashier
    • Present clearance from Security Officer for encoding of 201 file.
  8. Registrar's Office
    • Present receipt of payment. Get official registration form.



Enrollment Procedure for Shifting Course

  1. Registrar's Office
    • Secure shifting form.
  2. Home Dean
    • Present shifting form to be signed by the Home Dean. Request for a copy of summary of grades.
  3. Receiving Dean
    • Present shifting form to be signed by the Receiving Dean. Present the summary of grades for evaluation.
  4. Registrar's Office
    • Submit the accomplished shifting form for the change of course in the enrollment system.
  5. Dean's Office
    • Secure pre-enrollment form.
  6. Assessment Office
    • Present pre-enrolment form to be signed by the assessment officer.
  7. Library
    • Present pre-enrolment form to be signed by the librarian.
  8. SPS
    • Present pre-enrolment form to be signed by the guidance counselor.
  9. Dean's Office
    • Present signed pre-enrolment form. Request for validation and assessment of subjects to be enrolled.
  10. Cashier
    • Pay the assessed tuition fee and other fees.
  11. Registrar's Office
    • Present receipt of payment. Get official registration form.



Cross Enrollment Procedure (Inter-Department)

  1. Home Dean
    • Ask permission from your Home Dean for cross enrollment.
  2. Registrar's Office
    • Secure cross enrollment form from the Registrar's Office.
  3. Receiving Dean
    • Fill in the form and get the approval of the Receiving Dean where the subject is to be crossed-enrolled.
  4. Home Office
    • Have the cross-enrollment form signed by the Home Dean.
  5. Assessment Office
    • Present the pre-enrolment form to the assessment office representative (computation of tuition fee rate will be based on whichever department rate is higher).
  6. Cashier
    • Pay the assessed tuition fee and other fees.
  7. Registrar's Office
    • Present receipt of payment. Get official registration form.
