Is inherently a part of the administrative set-up of the institution and a member of the policy making body of the school.  His office is the repository of highly important, delicate, and confidential documents.  His decision carries weight in cases involving credentials, evaluation of subjects or credits, student accounting, academic placement, and eligibility for graduation.

Admission Requirements

  • Freshmen
    • F-138 (High School Report Card)
    • PEPT Certificate (original) (if applicable)
    • Certificate of Good Moral Character
    • Birth Certificate (NSO Authenticated)
    • Four (4) pcs latest ID picture (2x2)
    • Medical Certificate (for Maritime and HRM enrollees)
    • Drug Test result (for Maritime and Criminology enrollees)
    • Ishihara and Audiometry Test result (for Maritime enrollees)
    • Six (6) pcs. mail envelopes with stamps
  • Transferees
    • Certificate of Transfer
    • Copy of Grades for Evaluation
    • Official Transcript of Records (copy for UPHSL)
    • Certificate of Good Moral Character
    • Birth Certificate (NSO Authenticated)
    • Four (4) pcs latest ID picture (2x2)
    • Medical Certificate (for Maritime and HRM enrollees)
    • Drug Test result (for Maritime and Criminology enrollees)
    • Ishihara and Audiometry Test result (for Maritime enrollees)
    • Six (6) pcs. mail envelopes with stamps
  • Old Students
    • Clearance form duly signed by different department
    • Evaluation sheet signed by the Dean
    • Xerox copy of marriage contract if married (for female)
  • Second Courses
    • Certificate of Transfer
    • Official Transcript of Records (Graduated, Original)
    • Certificate of Transfer
    • Birth Certificate (NSO Authenticated)
    • Four (4) pcs latest ID picture (2x2)
  • Foreign Students
    • Alien Certificate Registration (ACR)
    • Student Visa/ Study Permit
    • Birth Certificate
    • Four (4) pcs. latest ID picture (2x2)
    • Copy of Academic Records from previous school
  • Policies on the Issuance of Records
    • Accomplished request form and appropriate clearance must be presented
    • Payment for pertinent fees
    • Except for Diploma and Transcript of Records, request must be released not later than one (1) week upon date of filing.
    • Documents must be released only to the person concerned.  In the event that the person cannot personally claim his requested document/s, a written letter of authorization and his identification card must accompany the claiming the documents.
  • Policies on Graduation
    • Only those students who complied and finished the academic and non-academic requirements will be eligible for graduation
    • Only those students who have no deficiency can join the commencement exercises.
